Job Description:

Bi-Weekly Hours: 75.0 hours bi-weekly / Days (evenings & weekends as required).(Commencing Immediately until September 2022)Reporting to the Provincial Renal Clinic Manager, the Renal Dietitian works with patients who have renal insufficiency and renal failure, along with their families, in promoting self-care and healthy behaviors, which will maintain quality of life and delay disease progression. This is performed through nutrition assessment including analysis of blood work, development and ongoing monitoring of nutrition care plans. Duties will include: - Assessing nutrition intake of patients with renal insufficiency and failure with the goal of developing a plan with the patient and their family to manage and monitor the nutrition intake and to maintain positive behaviors that promote optimal health for the patient;- Maintaining accurate patient records, reporting and communicating (verbal, written and electronic) nutrition assessment for dissemination to the Renal Program team members, and the primary care practitioner, as well as for statistical purposes;- Providing recommendations regarding the nutritional management of patients to the health care professionals on the renal team, this may include medication management, complications and treatment outcomes.- Providing individualized education to patients and their families on renal disease and nutrition in accordance with national standards, such as the Dietitians of Canada Standards;- Participating in quality improvement activities;- Participating in patient rounds;- Providing regular group education sessions on nutrition and the importance of physical activity as it relates to renal disease management with the goal of patients/families increasing their knowledge to make informed choices regarding food/fluid intake and activity;- Working collaboratively with the clinic manager to establish goals, objectives and protocol for the delivery, monitoring and evaluation of the renal dietitian services;- Planning, implementing and evaluating the nutrition and physical activity components of the renal education classes in consultation with other team members;- Working with Renal Program team members to develop appropriate educational materials, presentations and communication for individuals, groups and the general public;- Providing education on renal nutrition management to health care professionals,
community programs, health care centers, organizations and professional associations; participating in committees as required; and performing other related duties as required.
Minimum Qualifications: - Bachelor’s Degree majoring in Foods and Nutrition with a post-graduate internship in dietetics or a Master’s Degree in Human or Community Nutrition.- Registered or eligible for registration with College of Dietitians of Prince Edward Island.- Considerable recent experience in a related clinical nutrition setting (eg. Ambulatory care, diabetes, hypertension, cardiology, etc.).- Must have CPR Certification.- Ability to relate well with patients, staff, health professionals and the public with a pleasant and co-operative attitude, demonstrating sensitivity, empathy and warmth.- Ability to assess patient nutrition needs, analyze and make recommendations based on established guidelines.- Ability to exercise diplomacy, tact and judgment in dealing with difficult and complex situations.- Demonstrated proficiency in developing and presenting individual and group education.- Ability to work independently and as part of a team, while respecting flexible professional boundaries and team decisions.- Excellent communication skills and interpersonal ability to operate in a collegial environment with a wide range of health professionals, institutions, agencies and the public. - Ability to communicate effectively verbally, in writing and electronically.- Access to reliable transportation is required. - Ability to organize, develop, implement and evaluate education programs for patients, health professionals and the general public.- The successful applicant must have a good previous work and attendance record.- The successful applicant must provide a satisfactory criminal records check prior to beginning employment.Other Qualifications: - Previous experience in adult education would be an asset.- Recent experience in a Renal setting would be preferred.Please Note: Successful applicants are expected to complete the term of temporary assignment unless appointed to a permanent position.
